PRIVACY NOTICE

INTRODUCTION: BY INSTALLING, USING OR OTHERWISE ACCESSING THE DEMENTIA GUIDE EXPERT APPLICATION FROM ANY DEVICE, MOBILE OR OTHERWISE, YOU ARE AGREEING TO ABIDE BY THE TERMS AND CONDITIONS OF THIS PRIVACY NOTICE. IF YOU DO NOT AGREE TO THESE TERMS AND CONDITIONS, DO NOT DOWNLOAD AND/OR ACCESS THE DEMENTIA GUIDE EXPERT APPLICATION. BY USING THIS APPLICATION, YOU ARE CONSENTING TO THE COLLECTION, USE, AND DISCLOSURE OF INFORMATION IN ACCORDANCE WITH THIS PRIVACY NOTICE.

This Privacy Notice sets forth how your information is collected, used, transferred, and stored through the mobile application titled “Dementia Guide Expert” (“Application”) and the terms and conditions governing your use of the Application. The Application was created by the University of Illinois at Chicago and is owned by the Board of Trustees of the University of Illinois (“University”, “we”, “our”, “us”).

The purpose of this Application is to consolidate and provide the resources, tools and interventions to support caregivers and care-recipients while confronting the challenges of dementia. Our shared goal is to help the persons with dementia and their care partners to maximize their physical and mental functions in order to be able to continue to socialize and lead independent and purposeful lives. In addition, we hope to create a society better prepared for the challenges of dementia care and to encourage greater empathy and understanding in caring for persons with dementia.

This Privacy Notice is subject to the Application Terms of Service; the University’s Web Privacy Notice, which contains the following sections among others: Rules for Individuals under Thirteen Years of Age, Disclaimer of Warranties and Accuracy of Data, and Legal Notices of Terms and Conditions; and to the University of Illinois Supplemental Privacy Notice for certain persons in the European Economic Area and the United Kingdom (Supplemental Privacy Notice).

1. What information may the Application collect from you?

a) The Application requests that you complete a short survey indicating whether you 1) work in health care, 2) are a family member of a person with dementia, 3) are a person with dementia, or 4) “other” (i.e., a person who does not fit into the previous three categories). The survey also asks for your feedback on the usefulness of the Application. The University will not know the identity of anyone providing the requested information. By providing us with your information, you are consenting to our use of it in accordance with this Privacy Notice, the University of Illinois Web Privacy Notice, and the Supplemental Privacy Notice.

b) Automatic Collection of Non-Personal Information. The Application may collect certain non-personal information automatically that may not, on its own, permit direct association with any specific individual. The following are some examples of non-personal information that may be collected: usage, including frequency of access, duration, and time of access. By using the Application, you are consenting to the collection, use, and disclosure, of non-personal information by the Application.

2. Where will the information be stored?
The answers you provide to the survey questions will be stored in third-party cloud-based servers supporting the online survey tool used in the Application.

3. How is the information used?
To better understand the types of people using the Application, we will aggregate the survey responses we receive to determine the aggregate number of Application users who 1) work in health care, 2) are family members of persons with dementia, 3) are persons with dementia, or 4) who are “other”. We have no way of associating the survey responses we collect with specific individuals and we will not attempt to do so.

4. Is the information shared?
This Application was created by the University using grant funding provided by the Health Resources & Services Administration of the U.S. Department of Health and Human Services. The University will provide the Health Resources & Services Administration with the aggregate number of Application users who 1) work in health care, 2) are family members of persons with dementia, 3) are persons with dementia, or 4) who are “other”.

5. How is the information protected?
We follow generally accepted industry standards to protect the information submitted to us. No method of transmission, or method of electronic storage, is 100% secure. Therefore, we cannot guarantee its absolute security.

6. Do you have to respond to the survey questions?
You are not required to respond to the survey questions. The resources contained in the application will be available to you whether or not you answer any or all of the survey questions.

7. Changes to this Privacy Notice
We may amend this Privacy Notice from time to time. If we change this Privacy Notice we will post the changes on the University’s Mobile Application Privacy Policies and Terms of Service website. Your continued use of the Application after any such changes will constitute your acceptance of the updated Privacy Notice. If you do not wish to accept the revised Privacy Notice, you must not use or access (or continue to use or access) our Application. Please regularly check our Application or the Mobile Application Privacy Policies and Terms of Service website to determine if the Privacy Notice has been updated and to review those changes before deciding whether or not to access (or continue to access) the Application.
